 	<li>The test/calibration will be conducted about the abovementioned test standard/test method or based on the agreed test/calibration method.</li>

<li>LATL shall not be liable for whatever damage may occur on the test/calibration sample during testing/calibration.</li> 
 	<li>The customer shall be informed through email, phone call or in writing prior to deviations from testing/calibration work.</li> 
 	<li>Changes in the request for testing/calibration work should be communicated to LATL, writing, addressed to the Chief of the Laboratory.</li> 
 	<li>All personal data and transaction information required in this form will be solely used for the conduct of the service requested.</li> 
 	<li>The duration of the service is based on the updated LATL citizen’s charter available online: <a href="https://www.doe.gov.ph/citizens-charter" target="_blank" rel="noopener">https://www.doe.gov.ph/citizens-charter</a></li> 
 	<li>Upon signing this form, the customer read, understood, and agreed in the terms and conditions, as well as the <a href="#" data-toggle="modal" data-target="#genInfoModal">General Information in Acceptance of Work</a> in LATL written at the back page.</li>
